Scope: Last year the program had a total of 311 client visits Amount Granted: $1220 60th Ave Street Scape Project (Re-submittal)Submitted by: City of Sweet Home Purpose: Purchase a Sternberg Commercial Grade aluminum Light Pole. Scope: Unification of various geographic segments of the community Amount Granted: $2000 Digital Copier SystemSubmitted by: Sweet Home Emergency Ministries (S.H.E.M.) Purpose: Purchase a digital color laser copier with duplex capability, cartridges, replecement drum and 2 year extended service contract. Scope: Provide 1000+ copies per month. Amount Granted: $1505 Court Appointed Special Advocates (CASA) Volunteer ProgramSubmitted by: CASA of Linn County Purpose: Funds will be used to help recruit, screen and train new CASAs for Sweet Home and professionally support them for one year. Scope: Increasing service from four to and estimated 65 children in the Sweet Home area. Amount Granted: $2000 Advocacy ProgramSubmitted by: Linn COunty Child Victim Assessment Center (dba: ABC House) Purpose: Fund a portion of the cost of the Advocacy Program Scope: An estimated 30 children from the Sweet Home area are projected to require the services of ABC House. Amount Granted: $960 Vehicle Safety UpgradesSubmitted by: Fair Share Gleaners Purpose: Tires, brakes and alignment for organizations vehicles Scope: Fair Share Gleaners (as of the time of thise grant) currently serves 174 households of low-income people. Amount Granted: $2000 Power House Computer LabSubmitted by: Boys & Girls Club of Sweet Home Purpose: Purchase replacement computers for the Boys & Girls Club Scope: The Boys & Girls Club provides services to over 800 youth annually. Amount Granted: $1500 Senior Companion ProgramSubmitted by: Samaritan Health adn Samaritan Pacific Health Services Purpose: Service to seniors and disabled adults through 943 hours of service, 6250 miles of transportation service and 1351 shared meals - or a combinations of these services. Scope: In 2009 24 Sweet Home residents served or received services of the Senior Companion Program. Amount Granted: $2500 Senior Meals/Meals on WheelsSubmitted by: Oregon Cascades West COG Senior Services Foundation Purpose: Grant funds will be used to purchase meals for Sweet Home Citizens Scope: Assist in the service of 23,000 meals to Sweet Home residents Amount Granted: $2500 |
